Procedure for Processing of Change Program

Step1
Accomplish form here to request for Change Program.
Upload clear photo or scanned copy of the following documents:
1. 招生 Clearance, For Consideration Letter(for CONDITIONAL applicant), or Decline Letter
2. Grade 12 Report Card

Once change of program is approved, a new 招生 Clearance will be sent to you via email. If you do not see the email, check your “junk mail” folder or “spam” folder.
Step3
Pay the required down payment of Php3, 000.00. Head over here to view the list of accredited 8455新澳门路线网址 payment centers.
- IMPORTANT: Secure the new ADMISSIONS CLEARANCE first before making a down payment. The instructions for accomplishing the bank forms are indicated on the 招生 Clearance.
Step4
Proceed to the Online Enrollment Registration by heading over here.
- After submitting the online form, kindly wait for the email about the status of your enrollment registration. If successful, you will receive the login credentials to your Student Lifecycle Management and Information System (SLMIS) account. Class schedules and other related information can be viewed in the SLMIS.
